ABSTRACT

OBJECTIVE@#To testify the spatial relationship between the subscapularis muscle splitting window and the axillary nerve in modified arthroscopic Latarjet procedure, which could provide anatomical basis for the modification of the subscapularis muscle splitting.@*METHODS@#A total of 29 adult cadaveric shoulder specimens were dissected layer by layer, and the axillary nerve was finally confirmed to walk on the front surface of the subscapularis muscle. Keeping the shoulder joint in a neutral position, the Kirschner wire was passed through the subscapularis muscle from back to front at the 4 : 00 position of the right glenoid circle (7 : 00 position of the left glenoid circle), and the anterior exit point (point A, the point of splitting subscapularis muscle during Latarjet procedure) was recorded. The vertical and horizontal distances between point A and the axillary nerve were measured respectively.@*RESULTS@#In the neutral position of the shoulder joint, the distance between the point A and the axillary nerve was 27.37 (19.80, 34.55) mm in the horizontal plane and 16.67 (12.85, 20.35) mm in the vertical plane.@*CONCLUSION@#In the neutral position of the shoulder joint, the possibility of axillary nerve injury will be relatively reduced when radiofrequency is taken from the 4 : 00 position of the right glenoid (7 : 00 position of the left glenoid circle), passing through the subscapularis muscle posteriorly and anteriorly and splitting outward.

ABSTRACT

@#Objective     To compare the clinical effects of coronary artery bypass grafting (CABG) via the left anterior small thoracotomy (LAST) versus lower-end sternal splitting (LESS) approach in the treatment of coronary heart disease. Methods     The patients who underwent LAST CABG in Tianjin Chest Hospital from October 2015 to December 2020 were allocated to an observation group (LAST group), and the patients who underwent LESS CABG at the same period were allocated to a LESS group. Propensity score matching method was applied with a ratio of 1∶1. The baseline data, perioperative data and grafts data were compared between the two groups after matching. Results     Before matching, there were 110 patients in the LAST group, and 206 patients in the LESS group. After matching, there were 110 patients in each group. In the LAST group, there were 83 males and 27 females with an average age of 60.6±8.3 years. In the LESS group, there were 80 males and 30 females with an average age of 61.0±9.6 years. There was no statistical difference in baseline data between the two groups after matching (P>0.05). The hospital stay time (t=2.255, P=0.025) and ventilator using time (t=−2.229, P=0.027) in the LAST group were significantly shorter than those in the LESS group. There were no statistical differences between the two groups in the postoperative hospital stay time, ICU stay time, postoperative left ventricular ejection fraction, postoperative left ventricular end-diastolic diameter, average number of grafts, secondary intubation, secondary thoracotomy, postoperative wound infection, sternal complications, postoperative atrial fibrillation, postoperative pulmonary infection or main adverse cardiovascular and cerebrovascular events (P>0.05). There was no statistical difference in the distribution of target vessels in the anterior descending branch, diagonal branch or posterior descending branch between the two groups (P>0.05). The grafts of the LAST group were significantly more than those of the LESS group in the area of obtuse marginal branch and posterior ventricular branch, and the grafts of the LESS group were significantly more than those of the LAST group in the area of right coronary artery (P<0.05). Post-operative computerized tomography angiography indicated that 1 patient in the LAST group had obtuse marginal branch vein bridge vessel occlusion, and the bridge vessels in the other patients were unobstructed. Conclusion     Minimally invasive CABG via both LAST and LESS approaches is safe and effective. LAST approach can achieve complete revascularization for multi-vessel lesions, and it is safe and reliable, with the advantages of less trauma and aesthetic appearance. However, it requires a certain learning curve of surgical techniques and certain surgical indications.

ABSTRACT

Background: Dental Implant originally utilized for aesthetic and functional rehabilitation, this treatment option have transformed oral rehabilitation techniques and are now regarded as the gold standard of treatment for replacing single, partial, and full-arch teeth. With improvements in osteotomy technique, implant macro- and micro-geometry, surface treatment, types of implant prosthetic connections, and other aspects, the overall treatment duration has also been greatly shortened.Material & Methods:This study was carried out in two implant centers and is retrospective and descriptive. It was done in the Implant Surgery Centers of Banasree Dental and German Dental, Dhaka, Bangladesh. The research was carried out from January 2010 to June 2022. 63 people made up the entire sample for this study.Results:Most of the patients 21(33.3%) were aged between 51-60 years where most of the patients 36(57%) were female and 27(43%) were male. Maxilla was done in 45(71.4%) patients, mandible was done in 48(76.2%). 69(74.2%) implants were done on the anterior site and 24(25.8%) was done on posterior site. All the patients had a good primary stability of implant with an insertion torque of 30 N/cm or more. There were 100% survival rate after the implants and in good functional condition. All patients were happy with their implant.Conclusion:This approach of osteotomy preparation’s greater primary stability appears to have a minimal detrimental effect on implant success.

RESUMO: Categoria notável no panorama dos casos fronteiriços, a adicção sexual é uma configuração psicopatológica em que o ato sexual adquire teor irrefreável e nocivo, a despeito da compreensão intelectual e possíveis racionalizações do sex-addict sobre o problema. O objetivo deste artigo é investigar a primazia do mecanismo de defesa da clivagem no engendramento da adicção sexual, a partir de uma revisão metapsicológica e da apresentação de duas vinhetas clínicas. Destaca-se o caráter pseudo-reparador da atividade sexual que, sem êxito, visaria suturar fendas no sistema de um ego clivado, estagnado em suas capacidades integrativas e eróticas.


Abstract: A notable category in the panorama of borderline cases, sexual addiction is a psychopathological condition in which the sexual act acquires an irrepressible and harmful content, despite the sex addict's intellectual understanding and possible rationalizations about the problem. This article aims to investigate the primacy of the splitting defense mechanism in the engendering of sexual addiction, based on a metapsychological review and the presentation of two clinical vignettes. The pseudo-repair character of sexual activity stands out, which, without success, would aim to mend the cracks in the system of a split ego, hindered in its integrative and erotic capacities.

ABSTRACT

ObjectiveTo investigate the injury risk of simplified 24-action Taiji Quan wild horse mane splitting practice at different postures with mechanical analysis. MethodsThe mechanical model of the action of Taiji Quan wild horse mane splitting was establishied, and then the coordinates, velocity, acceleration and horizontal angle of thigh and lower leg were determined by image analysis. The force arm of quadriceps femoris was detected by X-ray, and the quadriceps femoris tension, femoral tibial joint force and patellofemoral joint pressure of wild horse mane splitting practice with different postures were compared. ResultsCompared with the high posture, the coordination and velocity decreased, the average angle of knee joint decreased from 128° to 115° at the low posture; and the knee moment, the quadriceps femoris tension, femoral tibial joint force and patellofemoral joint pressure significantly increased (F > 37.187, P < 0.001). ConclusionWhen practicing Taiji Quan wild horse mane splitting, the risk of knee injury at the low posture is higher than that at high posture.

ABSTRACT

In order to solve the problems of slow operation speed and low registration accuracy of thin plate spline (TPS) interpolation method for motion vector field in coronary computed tomography angiography (CTA), a multi-level B-spline interpolation method (MBS) with uniform grid is proposed. On the one hand, the interpolation method used local B-spline to refine the sparse mesh layer by layer in a multiscale way to improve the accuracy of registration. On the other hand, it used the splitting matrix method to interpolate the motion vector field, greatly reducing the operation time of interpolation. The experimental results show that the proposed method can be used for CTA image registration efficiently.

ABSTRACT

Objective:To investigate the effect of mandibular angle osteotomy on the morphological changes of mandible by using cone beam computed tomography (CBCT) scanning data and three-dimensional cephalometry.Methods:According to included and excluded criteria, the complete CBCT data of 20 female patients (aged 20-35 years with an average age of 25.5 years) from January 2016 to June 2018 who underwent curved mandibular angle osteotomy combined with splitting cortiectomy were selected. Seven items reflecting mandibular morphology before and after surgery were measured respectively. All the data were analyzed by the means of SPSS 12.0; linear correlation analysis and paired t test were used to evaluate the change of andibular morphology.Results:The measurement results of the mandible pre- and post-operative 10 days showed as follows: Go-Go decreased, both sides of Cd-Go, Go-Ri and MR-MA decreased, both sides of Go-Me became longer, both sides of Ar-Go-Me and MP-FH increased. The paired t-test results showed that there was significant difference before and after operation ( P<0.05), but there was no significant difference between the left and right measurement items ( P>0.05). Compared with that in 10 days after operation, the measurement items also changed 6 months after operation. There was no statistical difference between the above measurement items 6 months after surgery and 10 days after surgery ( P>0.05), and no statistical difference between the left and right sides ( P>0.05). Conclusions:The mandibular three-dimensional model which is reconstructed based on CBCT data, not only displays the three-dimensional anatomical structure that cannot be presented on X-ray film visually and comprehensively by means zoom, rotation and other operations, but also precisely and accurately shows the change of the shape of the mandible from the sagittal, coronal and horizontal plane with arbitrary cutting and rotation. The measurement of mandible is more accurate and reliable, and the surgical plan of mandibular angle osteotomy can be accurately planned preoperatively, and the operative effect can be quantitatively evaluated postoperatively.

ABSTRACT

Adequate bone volume is the primary condition for successful dental implants. However, sufficient bone volume is often encountered in the vertical direction, but the bone volume in the buccolingual direction is insufficient, making it less suitable to be implanted. If the traditional spitting technique is used in the mandible, fracture and necrosis can easily occur in the labial (buccal) bone plate due to the absence of elasticity, thick cortical bone, poor blood supply, and anastomotic branch. The two-stage ridge splitting technique can be used in patients with narrow alveolar ridge in the mandible. This study summarizes the principles and conditions of application, operational points, clinical efficacy, and analysis of the causes of buccal bone plate absorption.

OBJECTIVE: To discuss the technical requirements of scored tablets combining with the current situation of clinical use of scored tablets and the domestic and national technical requirements. METHODS: The FDA guidance on tablet scoring was introduced in detail, on this basis, the general considerations on the technical requirements of generic scored tablets were put forward. RESULTS: AND CONCLUSION: It was suggested that the evaluation criteria should be established on the basis of taking a full consideration on product characteristics in the process of research and development, scored tablets should be developed in accordance with clinical needs, and specification description should be standardized for clarifying the clinical use of the scored tablets.

Em um mundo onde a direita política ganha cada vez mais espaço, o psicanalista americano Christopher Bollas se coloca a pensar nas razões psíquicas desse movimento surpreendente. Para ele vivemos um problema psicopolítico, no qual a psicanálise tem a contribuir, inclusive emprestando léxico analítico ao contexto político. Explorando essa história psicopolítica desde o início do século XX, o autor propõe pontos de vista e padrões de pensamento que tentam contribuir na explicação do momento atual. Para ele, não mais somos oprimidos por um superego forte, mas, sim, por egos fracos e inoperantes, adaptativos às necessidades da sociedade. Nesse contexto, o splitting da personalidade, a projeção e a melancolia ganham espaço nas mentes e na política.


In a world where the political right gains more and more ground, the American psychoanalyst Christopher Bollas thinks about the psychic reasons of this surprising movement. Bollas believes we are experiencing a psycho-political problem, in which psychoanalysis has to contribute, including lending some analytical lexicon to the political context. Exploring this psycho-political history from the beginning of the twentieth century on, he offers points of view and thought patterns in order to contribute to a deeper explanation of the present moment. Bollas considers that we are no longer oppressed by a strong superego, but by weak and inoperative egos, adaptive to the needs of society. In this context, the splitting of personality, the projection and the melancholy gain space in people's minds and in politics.

ABSTRACT

BACKGROUND: In lumbar spinal stenosis, spinous process-splitting decompression has demonstrated good clinical outcomes with preservation of the posterior ligamentous complex and paraspinal muscles in comparison to conventional laminectomy, but the radiological consequence and clinical impact of the split spinous processes have not been fully understood. METHODS: Seventy-three patients who underwent spinous process-splitting decompression were included. The bone union rate and pattern were evaluated by computed tomography performed 6–18 months after surgery and compared among subgroups divided according to the number of levels decompressed and the extent of spinous process splitting. The bone union pattern was classified into three categories: complete union, partial union, and nonunion. The visual analog scale (VAS) score, Oswestry disability index (ODI), and walking distance assessed both before and 24–36 months after surgery were compared among subgroups divided according to the union pattern of the split spinous process. RESULTS: Overall, the rates of complete union, partial union, and nonunion were 51.7%, 43.2%, and 5.1%, respectively. In the subgroup with partial splitting of the spinous process, the rates were 85.7%, 14.3%, and 0%, respectively; those of the subgroup with total splitting of the spinous process were 32.9%, 59.2%, and 7.9%, respectively. With single-level decompression, a higher rate of union was observed compared with multilevel decompression. The VAS, ODI, and walking distance were significantly improved after surgery and did not differ according to the degree of union of the split spinous process. CONCLUSIONS: We found that the single-level operation and partial splitting of the spinous process were favourable factors for obtaining complete restoration of the posterior bony structure of the lumbar spine in spinous process-splitting decompression.

ABSTRACT

In order to meet the needs of the flow cytometry for the simultaneous analysis of multiple fluorescence wavelengths and small volume, the design method of flow cytometry spectrum analysis system is presented by analyzing the characteristics of Dyson structure. And according to the method, a flow cytometry spectrum analysis system is disigned with Dyson type.The system's spectral range is 400 nm to 800 nm, the defocused spot size is less than the pixel size 24μ mm, the ransfer function value is above 0.8 at the Nyquist cut-off frequency 21 lp/mm,the spectral resolution is less than 3 nm, and the overall size is 83.54 mm×85.60 mm.The system has good optical performance and small volume, which meets the needs of the flow cytometry fluorescence spectral analysis. The outstanding innovation of this system is the application of Dyson light splitting structure and EMCCD detector which is high speed and high sensitivity.

ABSTRACT

OBJECTIVE: To investigate the variety and frequency of oral tablets splitting commonly used in pediatrics of children’s hospital, and to explore the feasible scheme of tablet splitting, so as to provide the basis for clinical precision and individuation of drug use. METHODS: From 1st Jul. 2017 to 30th Nov. 2018, all the prescriptions of tablet varieties used for neonates hospitalized in internal and surgical departments in our hospital were obtained. The frequency of each tablet splitting was analyzed, and the literature reports of the tablets splitting were searched. The solubility, stability and recommended method of oral tablets splitting were analyzed, and the suggestion were made. RESULTS: In this study, there were 16 kinds of oral tablets that often needed to be divided into different doses, with a total frequency of 2 149 times. Vitamin B2, Levothyroxine sodium tablets and Ursodeoxycholic acid tablets ranked the top three in terms of dose splitting frequency, accounting for 80.3% of the total dosing cases. After consulting the literature, the implementation process of the dosage division scheme was formulated, and the splitting methods of commonly used tablets were summarized. It is suggested that clinical evaluation of tablet and patient characteristics should be taken to determine tablet dosing splitting plan. CONCLUSIONS: Tablet dosing splitting is common in neonatal medication in our hospital. The methods of temporary dispensing tablet into oral liquid drugs summarized in this paper can provide references when necessary to meet the needs of clinical individualized drug use.

ABSTRACT

O presente artigo tem como objetivo refletir sobre o conceito de moralidade em Winnicott a partir dos personagens Jean Valjean e Javert do livro Os miseráveis, de Victor Hugo. Jean Valjean, diante do desemprego e da fome de sua família, roubou um pão e foi preso por dezenove anos, pena agravada por quatro tentativas de fuga. Javert, um policial absoluto e intolerante que acompanhava Jean Valjean no presídio, encontrou o detento nas ruas e deu início a uma história de ataques, fugas, culpas, gestos de reparação e perdões. Sugere-se que o personagem Jean Valjean apresenta uma moralidade verdadeira, em que há uma preocupação com o outro, e Javert uma moralidade falsa, em que há uma cisão entre certo e errado. No mais, discute-se os efeitos dos dois tipos de moralidade na contemporaneidade.


This paper has as its goal to discuss the concept of morality in Winnicott using the characters Jean Valjean and Javert from the book Les Miserables by Victor Hugo. Jean Valjean, dealing with unemployment and his family starvation, stole a loaf of bread and was arrested for nineteen years. His penalty was worsened by four escape attempts. Javert, an absolute and intolerant policeman that escorted Jean Valjean in the jail, found the prisoner in the streets and began a story of attacks, escapes, guilt, reparation gestures and forgiveness. The character Jean Valjean is believed to have a true morality, in which there is a concern for others, and Javert a false morality, in which there is a split between right and wrong. Furthermore, the effects of the two types of morality in nowadays are discussed.

Objective@#The present study evaluated postoperative reactions in adjacent teeth after the extraction of lower-level horizontal-impacted mandibular third molars using a T-shaped splitting method. @*Methods@#A total of 101 patients with lower-level horizontal impaction of mandibular third molars were selected and randomly divided into two groups. The impacted mandibular third molar was extracted using a T-shaped splitting method in group A (56 cases), and the traditional two-section method was used in group B (45 cases)。 Postoperative reactions, including temperature sensitivity, percussion pain and mobility in the adjacent teeth, were compared between the 2 groups at 1-week, 2-week, and 1-month follow-up visits. The SPSS 13.0 software package was used for the statistical analysis.@*Results @#The postoperative reactions of temperature sensitivity (χ21w= 11.81, χ22w = 17.43, P < 0.05), percussion pain (χ21w = 8.70, χ22w = 4.75, P < 0.05) and mobility (χ21w = 4.50, χ22w = 7.10, P < 0.05) in adjacent teeth in group A were significantly less than those in group B at the 1-week and 2-week follow-up visits. The temperature sensitivity in adjacent teeth in group A was significantly less than that in group B at the 1-month follow-up visit (χ2 = 7.10, P < 0.05), but percussion pain and mobility in adjacent teeth disappeared in both groups. @*Conclusion@#The T-shaped splitting method for the extraction of mandibular lower-level horizontal impacted third molars can reduce the postoperative reactions in adjacent teeth.

ABSTRACT

The research for efficient and low-cost electrocatalysts for water splitting are essential for the exploitation and application of hydrogen energy. Transitional metal phosphides are considered as one of the most promising bifunctional water splitting electrocatalysts. Herein, we reported a facile two-step method (firstly hydrothermal preparation, then phosphorization under low temperature) to synthesize the bead-chain like nanoarrays of CoP supported on three dimensional Nickel foam (CoP/NF). The as-prepared CoP/NF could act as an efficient bifunctional catalyst for overall water splitting. The catalyst exhibited remarkable activity for both the oxygen evolution reaction (OER) and hydrogen evolution reaction (HER) in alkaline media,delivering a current density of 10 mA/cm2at an overpotential of 281 mV for OER and 95 mV for HER, respectively. Efficient water splitting in alkaline system was realized by applying a two-electrode system with the CoP/NF acting as both the anode and cathode. The applied potential was 1.63 V to obtain the current density of 10 mA/cm2,and good stability was also testified.

La causa más frecuente y conocida de hiperamoniemia es la alteración en la función hepática. Sin embargo, se deben considerar otras patologías, de menor frecuencia y poco conocidas. La infección del tracto urinario por gérmenes productores de ureasa debe ser contemplada a pesar de ser infrecuente en pediatría. Se reporta el caso de un niño con encefalopatía aguda grave, con niveles elevados de amonio en sangre, en quien, luego de descartar otros diagnósticos diferenciales, se asumió el cuadro como hiperamoniemia secundaria a infección del tracto urinario por Corynebacterium riegelii, un germen productor de ureasa. Se implementaron medidas generales de tratamiento para la encefalopatía hiperamoniémica y tratamiento antibiótico específico, con buena evolución el paciente.


Elevated level of ammonia in the blood, defined as hyperammonemia, is feasible to cause neurological symptoms when crossing the blood-brain barrier. The most frequent and studied cause of hyperammonemia is liver failure. Nevertheless, other less frequent and known etiologies must be considered. Urinary tract infection caused by urea-splitting bacteria, despite being unusual in pediatric patients, must be taken into account. We report a pediatric patient with severe acute encephalopathy and high levels of ammonia in blood. After ruling out other causes of hyperammonemia, it was assumed secondary to urinary tract infection by Corynebacterium riegelii, a ureasplitting bacteria. General treatment for hyperammonemic encephalopathy was established, as well as specific treatment with antibiotics. The patient evolved favorably.

Partindo da temática Exílios e repatriações, a autora irá considerar as rupturas precoces no psiquismo e a defesa self falso, que pela cisão mantém o self verdadeiro em exílio. Indaga se a busca do verdadeiro self, objetivo da análise, segundo Winnicott, pode ser considerada uma forma de repatriação, e examina então os significados de self, exílio e pátria. Aborda também as possíveis formas de sutura promovidas pela experiência analítica e suas extensões.


Partiendo de la temática Exilios y repatriaciones, la autora considerará las rupturas tempranas en el psiquismo y la defensa self falso, que, a través de escisión, mantiene el self verdadero en exilio. Indaga si la búsqueda del verdadero self, objetivo del psicoanálisis, según Winnicott, podría ser considerada una forma de repatriación. Examina, entonces, los significados de self, exilio y patria. Aborda también las posibles formas de sutura promovidas por la experiencia analítica y sus extensiones.


Starting from the current issue's theme, that is Exiles and repatriations, the author will consider premature ruptures in the psyche and the defense of the false self ­ a false self that keeps the true self in a state of exile through the ego defense mechanism of splitting. According to Winnicott, the pursuit of the true self is the purpose of psychoanalysis. This paper questions whether this pursuit may be considered a way of repatriation. As such, the author examines the meaning of self, exile, and homeland (or fatherland). This paper also studies possible sorts of sutures which may be provided by the psychoanalytic experience and its extensions.
